Techniques and results of off-pump coronary artery bypass grafting using homemade intracoronary shunt.

摘要: BACKGROUND The outcome of the off-pump coronary artery bypass grafting (CABG) is highly dependent on surgical techniques. To overcome problems intraoperative hemodynamic instability and avoid potential injury to arising from occlusive technique, authors have modified strategy performing distal anastomosis by using authors' homemade intracoronary shunt. OBJECTIVE document results CABG characteristics shunt techniques are demonstrated. MATERIAL AND METHOD constructed silicone tubing (Allied Biomedical Company, Paso Robles, CA). It has been successfully used for anastomoses in surgery. All 170 consecutive patients who had October 2002 May 2005 were evaluated. Data collected retrospectively. Average grafts per patient 3.5. Arterial most cases (90%). Conversion rate was 2.8%. Patients followed up at 3 weeks, 6 bimonthly thereafter. RESULTS In a 23-month follow-up, operative mortality 0.5% (one patient). Complications included stroke (1.8%) post-operative AF 12 (7.12%). Of patients, (7.05%) required or postoperative intraaortic balloon pump support. early good satisfactory. CONCLUSION Off-pump can be performed with better control. safe, simple, reliable, effective.
