摘要: We report a case of 37-year-old man with Maturity Onset Diabetes the Youth (MODY) type 5, admitted for an episode cholestasis and simultaneous acute kidney injury (AKI). Chronic liver disease was due to mutation in transcription factor 2 (TCF2) gene, thus highlighting need close follow-up these patients. AKI attributed cholemic nephropathy based on following rationale: (1) alternative diagnoses were actively ruled out; (2) onset coincided severe hyperbilirubinaemia; (3) renal pathology showed large bile tubular casts marked necrosis (4) creatinine serum dramatically decreased when bilirubin levels improved after first sessions extracorporeal albumin dialysis (ECAD), suggesting its role recovery. Even though can precipitate injury, diagnosis could require biopsy at times. Future studies should confirm benefits ECAD nephropathy.
