Effective Use of Sertraline for Pathological Laughing after Severe Vasospasm Due to Aneurysmal Subarachnoid Hemorrhage: Case Report

摘要: Pathological laughing, one subgroup of psuedobulbar affect, is known as laughter inappropriate to the patient's external circumstances and unrelated internal emotional state. The authors present case a 76-year-old woman with no significant medical history who experienced pathological laughing after subarachnoid hemorrhage (SAH) due rupture an aneurysm, which was successfully treated craniotomy for aneurysm clipping. In acute stage operation she suffered from severe vasospasm resulting middle cerebral artery territory infarction conscious disturbance. As regained consciousness afflicted by 6 months onset SAH. Her involuntary situation incongruent state, could not control herself. Finally diagnosis made treatment sertraline, selective serotonin reuptake inhibitor (SSRI), effectively cured symptoms. estimated be consequence in right prefrontal cortex and/or corona radiata, vasospasm. To authors' knowledge, this first report aneurysmal offer insight into pathophysiology rare phenomenon. Effectiveness sertraline would widen modality against laughing.
