摘要: Septic shock is characterised by a decrease in systemic vascular resistance. Nevertheless, regional increases resistance can occur which may predispose to organ dysfunction, including the adult respiratory distress syndrome (ARDS). Because endothelial damage major feature of acute lung injury, we examined whether potent vasoconstrictor peptide endothelin-1 plays pathophysiological role sepsis ARDS. Plasma endothelin was measured mixed venous, pulmonary capillary and arterial blood, relationship with outcome measures determined. The intensive care unit university teaching hospital. A consecutive series well-characterised patients syndrome, both (n=11) without (n=15) ARDS, ventilated controls or ARDS (n=7). significantly elevated alone did not differ among blood. On multiple regression analysis, plasma correlated positively failure score oxygen consumption, negatively PaO2∶FiO2 ratio. There no correlation creatinine, suggesting that decreased renal clearance account for high concentrations. Although does appear be site production critically ill sepsis, increased contribute vacular resistance, hypoperfusion, development failure, sepsis.
