The Terminal Portion of the T Wave: A New Electrocardiographic Marker of Risk of Ventricular Arrhythmias

摘要: Experimental studies have shown that transmural dispersion of repolarization (DoR), defined as the difference in action potential duration between mid-myocardial M-cells, epicardial, and endocardial cells is reflected terminal portion T wave (TpTe) on surface ECG. Since DoR an important factor associated with propensity for reentrant arrhythmias, this study examined if TpTe may serve a marker risk ventricular arrhythmias. Data from 18 patients coronary artery disease inducible sustained tachycardia (VT group) were compared those 16 survivors myocardial infarction without VT (control group). was automatically measured each beat 24-hour ECG recordings, programmed stimulation performed antiarrhythmic drug-free state. expressed absolute interval milliseconds, relative to QTe (TpTe/QTe x 100%). 74 +/- 14 ms group versus 63 control (P < 0.004). The percent QT 21 4% 17 3% = 0.02). In disease. longer with, without, VT. results support hypothesis reflects repolarization.
