摘要: The assessment of the hypothalamic-pituitary-adrenal axis in patients with chronic renal failure (CRF) on hemodialysis is often hampered by abnormal responses to standard 1-mg dexamethasone suppression test. Various mechanisms have been proposed explain this lack suppressibility. present study was designed look into possible for these findings CRF. We studied 6 CRF and 5 healthy subjects utilizing test as well 50-mg hydrocortisone Samples were assayed dexamethasone, adrenocorticotropic hormone, corticosterone, cortisol both direct radioimmunoassay (RIA) RIA after paper chromatography. Utilizing RIA, 4 exhibited blunted suppression, while all showed normal suppressibility when measured In contrast, controls regardless assay procedure used. unreliable setting Mean levels similar groups. Plasma hormone significantly higher patients, possibly indicative an underlying abnormality. Abnormalities testing may be explained overestimation rather than alteration absorption or metabolism. Measurement chromatography superior
