Development of a brief self-report questionnaire for screening putative pre-psychotic states.

摘要: Abstract Background Several self-report instruments were developed to capture psychotic prodrome, and claimed have good predictive validity. The feasibility of screening is questionable considering the heterogeneity targeted populations negative ramifications false positive identification. This study a questionnaire using data covering wide range clinical characteristics. Methods One hundred eleven putative pre-psychotic participants, 129 normal comparison subjects, 95 non-psychotic psychiatric outpatients completed 231-item comprising 110-item Wisconsin prone scale, 74-item schizotypal personality questionnaire, 33-item basic symptoms, 14-item cognitive symptoms. Items showing best discriminating power, estimated chi-square statistics with Bonferroni correction, extracted create brief version. A two-stage cut-off approach emphasizing specific items was applied maximize sensitivity specificity. concurrent validity proposed ten-fold cross-validation procedure. Results 15-item developed. Respondents checking at least eight items, or those three seven including any referring feeling stress in crowds, aloofness, perceptual disturbance, would be considered putatively largest sensitivity + specificity (0.784 + 0.705 = 1.489). selection estimate by calculating 1000 permutations Conclusions investigation proposes different orientation for applying questionnaires screen states, less emphasis on attenuated symptoms values. Besides providing handy tool increasing awareness referral, instructions such should carefully worded.
