摘要: Abstract Background Family planning programmes in developing countries need a better understanding of nurse–patient communication order to improve the quality counselling. Objectives To identify factors clinic and community that enable nurses patients communicate effectively with one another. Design The study explored personal experiences who especially during family consultations (so-called "positive deviants"). Setting Sixty-four randomly selected public clinics located East Java, Indonesia. Participants Seven positive deviant 32 were identified from among 64 768 participated an earlier patient coaching study. Flooding prevented 5 participating study, reducing their number 27. Methods Investigators conducted: (1) content analysis qualitative data collected by structured in-depth interviews focus-group discussions (FGDs) patients, (2) analyses variance (ANOVA) quantitative on clinic, nurse, characteristics. Results Positive four factors, listed rough importance, helped them effectively: independent strengthen knowledge skills; aids; feedback colleagues; motivation stemming desire help people, patients' appreciation, husband's support, increased income. five enabling factors: due for service; confidence own nurses; belief right responsibility aids. Conclusions Insights suggest efforts should go beyond conventional skills training. Managers consider mix clinic-based interventions (such as peer feedback, aids, management flow) community-based education mass media).
