Copeptin concentration in cord blood in infants with early-onset sepsis, chorioamnionitis and perinatal asphyxia

摘要: Background: Vasopressin is one of the most important physiological stress and shock hormones. Copeptin, a stable vasopressin precursor, promising sepsis marker in adults. In contrast, its involvement neonatal diseases remains unknown. The aim this study was to establish copeptin concentrations neonates different states such as sepsis, chorioamnionitis asphyxia. Methods: Copeptin cord blood concentration determined using BRAHMS kryptor assay. Neonates with early-onset (EOS, n = 30), (n 33) asphyxia 25) were compared control group preterm term 155) neonates. Results: Median 36 pmol/l ranging from undetectable 5498 (IQR 7 - 419). non-normally distributed increased gestational age (p < 0.0001). born after vaginal cesarean delivery had elevated levels correlated strongly umbilical artery pH (Spearman’s Rho -0.50, p 0.0001), base excess (Rho -0.67, 0.0001) lactate at NICU admission 0.54, No difference found when comparing between EOS controls (multivariate 0.30). highest (median 993 pmol/l). Receiver-operating-characteristic curve analysis showed that associated asphyxia: area under resulted 0.91 (95%-CI 0.87-0.96, A cut-off 400 sensitivity 92% specifity 82% for defined study. Conclusions: related factors perinatal birth acidosis, delivery. appears be unsuitable diagnosis EOS.
