摘要: Obstetric cholestasis has been associated with a high incidence of stillbirth and perinatal complications. Between 1975 1984, 83 pregnancies were complicated by cholestasis. Meconium staining occurred in 45%, spontaneous preterm labour 44%, intrapartum fetal distress 22%. Of 86 infants two stillborn one died soon after birth. Perinatal mortality fell from 107 previous series this hospital (1965-1974) to 35/1000 series. Cardiotocography, estimations oestriol, liver function tests ultrasonic assessment amniotic fluid volume failed predict compromise, whereas amniocentesis revealed meconium 8 26 pregnancies. Early intervention was indicated 49 pregnancies, 12 because compromise. This study suggests that intensive surveillance, including for meconium, induction at term or mature lecithin/sphyngomyelin ratio, may reduce the rate 'high-risk' condition.
