Blood pressure crisis following withdrawal of clonidine (Catapres, Catapresan), with special reference to arterial and urinary catecholamine levels, and suggestions for acute management

摘要: Abstract In five patients with severe essential hypertension, placebo was substituted after 24 to 48 months of treatment clonidine and a diuretic. the present study, four developed marked blood pressure rise following withdrawal which rapidly reversed by intravenous administration propranolol 0.2 mg. per kilogram body weight phentolamine 20 30 The fifth patient pretreated reserpine 2.0 intramuscularly for three days prior clonidine, he experienced far less impressive. All similar “withdrawal” symptoms, consisting headaches, insomnia, restlessness, tremor, nausea. Catecholamines were determined in arterial urine, before during overshoot. They revealed increase, particularly urine samples, consistent appearance hyperadrenergic state.
