Left subclavian artery management in endovascular repair of thoracic aortic aneurysms and aortic dissections.

摘要: Purpose: The purpose is to report our experience and revise previously published results in endovascular repair of short-necked thoracic aortic aneurysms or type B dissections, which the left subclavian artery (LSA) was occluded by stent graft intentionally. Methods: Seven patients with an dissection three who had a aneurysm were treated endovascularly grafts. In all ostium LSA graft, only two primary, prophylactic revascularization performed transposition common carotid (LCA). Two types grafts used: Talent® (Medtronic) Excluder® (Gore) graft. Results: sealing entry tear dissections exclusion existing achieved. No immediate neurological deficit arm ischemia occurred. Nevertheless, during mean follow-up 18 months (2 31 months) second surgical intervention be due steal syndrome, ischemia, continuing perfusion dissected false channel. Conclusion: Intentional occlusion stent-graft diseases seems safe procedure. Close needed arising persistent channel via after patients' discharge, requiring intervention.
