Endoscopic endonasal transsphenoidal surgery for invasive pituitary adenoma.

摘要: Invasive pituitary adenomas (IPA) involving the skull base extend from sella region, and invade surrounding structures. In present study, we reviewed therapeutic efficacy in a group of patients with IPA treated endoscopic endonasal transsphenoidal surgery. Data 78 at our hospital were retrospectively reviewed. The diagnostic modalities, surgical techniques, outcomes Diagnosis was confirmed by endocrinological profile CT or MRI all patients. Surgery performed via an approach. Thirty-five (44.9%) had hormonally active tumors, 43 (55.1%) nonfunctioning tumors. Complete removal tumor achieved 62 (79.5%) subtotal 12 (15.4%); partial remaining four (5.1%) who fibrous dumbbell-shaped adenomas. mean follow-up 43.2 months 65 clinical symptoms improved to varying degrees. 52 patients, tumors completely disappeared on imaging. Visual 96.4% presented visual impairment. These results show that surgery for resection has advantages. We suggest method is safe, minimally invasive efficient technique IPA, providing good visualization operative field, generally complete removal, short procedure duration, minimal postoperative complications.
