摘要: Hydroxyethyl starch (HES) solutions are commonly used for volume replacement in cardiac surgery patients. The degree of impairment the haemostatic system depends on molecular weight and substitution HES solutions. It is claimed that as 130/0.4 (Voluven(®)) exhibits a lower vitro hydroxyethyl than 200/0.5 (HAES-steril(®)) therefore it has less impact haemostasis. A best evidence topic was written according to structured protocol verify this statement. question addressed was: patients does with Voluven(®) impair coagulation other colloids? Using reported search 12 papers, three nine clinical studies, were found represent answer question. studies all randomised controlled trials. authors, journal, date country publication, patient group studied, study type, relevant outcomes results these papers tabulated. suggest no significant effect platelet variables, shows faster clot formation process better retraction compared On hand, current available (level 1b) from limited by heterogeneity predominantly terms dosage administered sample size individual trials, overwhelmingly suggests or gelatin-based fluid affects same extent resulting similar blood loss. can be concluded contrary practice comparable effects loss after surgery.
