Malignant degeneration of pilonidal cyst rare, with incidence estimated at 0.1%. The most type squamous cell carcinoma and the treatment choice remains en block resection lesion. Case Report: We present case patient locally advanced arising in cyst, due to misdiagnosis during his first treatment. Conclusions: Detailed histological examination all excised lesions essential any suspicion should prompt wider excision.
