Ventricular tachycardia of left bundle branch block configuration in patients with isolated right ventricular dilatation. Clinical and electrophysiological features.

摘要: Electrophysiological studies showed ventricular tachycardia in five patients (four male, one female) with isolated right dilatation. All had been asymptomatic before the onset of palpitation which developed adolescence or early adult life. Tachycardia associated syncope four patients, and three resuscitated from fibrillation investigation. The electrocardiogram during a left bundle branch block pattern, endocardial mapping at electrophysiological study confirmed origin. presenting could be induced all by programmed stimulation, differing configuration induced, but origin pattern were maintained. In two degenerated into fibrillation. Cineangiography, cross sectional echocardiography, multigated radionuclide angiography dilated abnormal ventricle while indicating that function was normal. On resting electrocardiograms T wave inversion over precordial leads sole abnormality. There no signs heart failure exercise tolerance Four have received maintenance treatment antiarrhythmic drugs, undergone operative ablative surgery. Thus complicating dilatation may serious symptoms electrical instability; adults it suspected on clinical grounds inverted waves leads.
