Successful implementation of clinical practice guidelines for pressure risk management in a home nursing setting.

摘要: Rationale  This paper reports an initiative which promoted evidence-based practice in pressure risk assessment and management among home nursing clients Melbourne, Australia. Aim objectives  The aim of this study was to evaluate the introduction uptake Australian Wound Management Association Guidelines for Prediction Prevention Pressure Ulcers. Method  In 2007 a pilot conducted. Nurse perspectives (n=21) were obtained via survey client profile (n=218) generated. Audit continued use screening tool, during later once implemented as standard organizational wide, conducted. Results  Nurses at site successfully guidelines, adopted supporting resources well received. Most low ulcer development. maintained extended their success, ensuring more than 90% screened over 18 months followed. All other sites performed less initially, however subsequently improved, meeting success after months. Two years later, organization continues screen all risk. Conclusion  Implementation clinical guidelines successful project became well-adopted practice. Success following wide implementation. Pilot findings suggest it may be prudent monitor status so prevent increasing development group.
