Clinical efficacy of three different minimally invasive procedures for far lateral lumbar disc herniation.

摘要: BACKGROUND Far lateral lumbar disc herniation (FLLDH) accounts for 2.6% to 11.7% of all herniated discs. Numerous surgical approaches have been described treating this condition. The purpose study was evaluate minimally invasive techniques the treatment FLLDH. METHODS From June 2000 March 2006, 52 patients with FLLDH were treated procedures. All assessed by anteroposterior and roentgenography computed tomography (CT). Some underwent myelography, discography, magnetic resonance imaging. Procedures performed included Yeung Endoscopy Spine System (YESS) (n = 25), METRx MicroDiscectomy 13), X-tube 14). Patients followed up a mean 13.5 months. Clinical outcomes using visual analog scale (VAS) Nakai criteria. RESULTS 3 procedures significantly improved radiating leg symptoms (P < 0.005). After surgery, 84.0%, 84.6%, 92.8% in YESS, METRx, groups had excellent or good outcomes. There no statistically significant differences VAS scores between groups. YESS procedure associated shortest operation time, simplest anesthesia, least trauma compared other 2 procedures, especially type I herniations. most suitable II herniations posterior endoscopic facetectomy. Posterior interbody fusion unilateral pedicle screw instrumentation combined degenerative instability (type III). CONCLUSION Minimally strategies options should be determined reference
