摘要: Colonic resection leads to insulin resistance, but the mechanisms are unknown. We used an integrated approach examine adipose tissue and skeletal muscle metabolism in patients lacking a colon. Ten healthy colectomized having undergone surgery for ulcerative colitis 10 matched control subjects were studied with hyperinsulinemic-euglycemic clamp measure sensitivity, arteriovenous sampling meal tolerance study postprandial substrate flux across muscle, biopsies quantify expression of genes involved glucose lipid metabolism. Colectomized exhibited lower sensitivity (homeostatic model assessment model, 33% reduction, P = 0.03; minimal 29% 0.05), elevated aldosterone (9-fold, 0.003), leptin (2.2-fold, 0.03), increased rate nonesterified fatty acid glycerol release from (P 0.02) especially late period. The uptake acids into was also significantly 0.007), as CD36 LPL mRNA compared controls. In tissue, hormone-sensitive lipase 0.015), whereas peroxisome proliferator-activated receptor-gamma decreased 0.02), that 0.001). this study, alterations after colonic altered may have contributed impairment sensitivity.
