The utility of peripheral thyrotropin receptor mRNA in the management of differentiated thyroid cancer.

摘要: Background Our aim was to analyze the utility of peripheral thyrotropin receptor (TSHR) messenger RNA (mRNA) in predicting and detecting recurrence differentiated thyroid cancer. Methods Peripheral blood TSHR-mRNA obtained 103 patients before after total thyroidectomy. An analysis performed correlate concentration with oncologic outcomes. Results Tumor types were papillary ( n  = 92), follicular  = 9) Hurthle cell  = 2) Preoperative ≥1.02 ng/μg 85% (88/103). On follow-up (median 48 months), 10 (10 %) developed recurrence. Recurrence rate a preoperative ≥ 1.02 ng/μg 11% versus 0% those lesser concentration. correctly diagnosed 7 (70%) recurrences. Of 19 positive thyroglobulin (Tg) antibodies, confirmed disease-free status 12 (63%) 1 (5%). For Tg, whole-body radioactive iodine scan, sensitivity 70%, 75%; specificity 94%, 76%, 97%; PPV 54%, 24%, 67%; NPV 97%, 96%, 98%, respectively, recurrent disease. Conclusion This study shows that may be at greater risk for compared In presence Tg accurately predicted disease 68% patients. Its overall performance similar albeit lower PPV.
