Outcomes of Synthetic Mesh in Contaminated Ventral Hernia Repairs

摘要: Background Given the questionable long-term durability of biologic meshes, additional prosthetic options for ventral hernia repairs (VHR) in contaminated fields are necessary. Recent evidence suggests improved bacterial resistance reduced-weight, large-pore synthetics, giving a potential mesh alternative repair hernias. We aimed to evaluate clinical outcomes 2 institutions' experience implanting lightweight polypropylene synthetic clean-contaminated and fields. Study Design Open VHRs performed with retro-rectus position were evaluated. Primary parameters included surgical site infection, occurrence, removal, recurrence. Results One hundred patients (50 male, 50 female) mean age 60 ± 13 years body mass index (calculated as kg/m ) 32 9.3 met inclusion criteria. There 42 58 cases. The incidence occurrence was 26.2% cases 34% 30-day infection rate 7.1% 19.0% total 7 recurrences follow-up 10.8 9.9 months (range 1 63 months). Mesh removal required 4 patients: due early anastomotic leaks, stomal disruption retraction morbidly obese patient, from enterocutaneous fistula. Conclusions Although perhaps not yet considered standard care United States, we have demonstrated favorable recurrence, rates associated use VHR.
