Glucose Hypometabolism and Neuropathological Correlates in Brains of Dementia with Lewy Bodies

摘要: Abstract Cerebral glucose metabolism using positron emission tomography (PET) with 18F-fluorodeoxyglucose was examined in 11 patients probable Alzheimer's disease (AD), 6 probable, and 1 patient autopsy-confirmed dementia Lewy bodies (DLB) as well 10 age-matched normal control subjects. Among widespread cortical regions showing hypometabolism the DLB group, metabolic reduction most pronounced visual association cortex compared to that AD group. Using a ratio of 0.92 cutoff (mean-2 SD subjects), could be distinguished from sensitivity 86% specificity 91%. In contrast, apolipoprotein E4 allele frequency cerebrospinal fluid tau levels did not differ significantly between two groups. order further dissect out neuropathological correlates dysfunctional occipital lobe, postmortem brains 19 17 controls were examined. A distinct extensive spongiform change coexisting gliosis variably noted throughout cerebral white matter relative sparing gray DLB. Notably, prominently consistently found region DLB, severity each brain generally paralleled regional difference reduced living patients. These findings suggest (1) among several potential antemortem biomarkers diagnosis measures may an informative diagnostic aid distinguish AD; (2) pathological process generates long projection fibers contribute, at least part, characteristic imaging features
