摘要: Abstract PURPOSE: Orthostatic hypotension is common among the elderly, but its relation to falls not certain. We determined whether orthostatic hypotension, including timing and frequency, was associated with in elderly nursing home residents. SUBJECTS AND METHODS: conducted a prospective study of 844 (60 years age older), long-stay residents at 40 facilities that were part multistate chain. All subjects able maintain weight-bearing for least 1 minute. defined as 20 mm Hg or greater decrease systolic blood pressure from supine standing, measured after 3 minutes standing on four occasions (before breakfast, before lunch). The outcome any subsequent fall during mean 1.2 follow-up. RESULTS: present (at one measurement) 50% falls. However, history previous past 6 months, those had an increased risk recurrent [adjusted relative (RR) = 2.1; 95% confidence interval (CI), 1.4 3.1]. greatest fallers who two more measurements (RR 2.6; CI, 1.7 4.6). association between independent demographic clinical factors meals) did affect CONCLUSIONS: factor Although benefit treating will require further study, it may be prudent identify high-risk institute precautionary measures.
