Acid-base parameters for predicting magnetic resonance imaging measures of neurologic outcome after perinatal hypoxia-ischemia: is the strong ion gap superior to base excess and lactate?

摘要: We conducted this study to compare the strong ion gap (SIG) with base excess (BE) and lactate for predicting neurologic outcome measured by magnetic resonance imaging (MRI) in newborns hypoxic-ischemic encephalopathy (HIE). In a retrospective cohort of 39 HIE treated whole-body surface cooling (n = 17) no 22), we blood SIG, BE, at 4, 24, 48 hours after birth, determined cerebral injury severity T1-, T2-, diffusion-weighted MRI scores age 5 days. Lower SIG levels correlated better outcome. The highest correlation coefficient (0.63) was "no cooling" subcohort between 24 hours; latter also had area under receiver operating characteristic curve (AUC), 0.90, positive negative predictive values 84 90%. outperformed subcohort, vice-versa "cooling" subcohort. All BE AUCs were <0.6. Overall, is similar as prognostic parameter. birth do not predict While displacing an additional parameter first 2 days hypoxia-ischemia.
