摘要: Of people living with HIV in the US, ~16 % or over 150,000 individuals passed through a correctional facility 2006. Given enormous impact of within incarcerated populations, facilitating continuity care from jails to community is particularly important reducing morbidity and mortality for releasees. Grantees participating Enhancing Linkages Primary Care Jail Settings Initiative developed models identifying HIV-positive detainees during incarceration linking them following release. In this sample 1,021 HIV-infected releasees, 79 received clinical services 74 additional 30 days post-release. Our analysis found several significant factors associated linkage including: receipt medication education jail, having completed discharge plan at release, staff awareness clients’ release date, stable housing on 30th day addition, subset participants who had both jail viral load assessments showed statistically increase suppressed load. EnhanceLink data suggest that may be effective settings engage care.
