Outcome evaluation of an intervention to improve the effective and safe use of meropenem

摘要: Background Pharmacists have been involved in promoting the proper and safe use of antimicrobial drugs our institution since 2010. Setting Kochi Medical School Hospital, Japan. Objective To design evaluate a plan administration meropenem (MEPM) based on its pharmacokinetics pharmacodynamics, drug sensitivity, bacterial cultures, patient condition renal function. Method A total 547 patients admitted between April 2010 March 2013 with serious infections who were successfully treated MEPM for three or more days analysed. Patients initially divided into two groups according to function: group consisted mild dysfunction [creatinine clearance (CLcr) > 50 mL/min] while B moderate severe (CLcr ≤ mL/min). These then subdivided implementation pharmacist intervention. Main outcome measures Daily dose, frequency administration, dose interval, duration therapy, adverse events cost reduction.
