Preservation of hearing in surgery for acoustic neuromas

摘要: Preservation of hearing was attempted in 161 cases histologically confirmed acoustic neuroma removed by the senior author between January 1, 1970, and September 30, 1991. There were 136 patients with unilateral tumors; 22 had bilateral tumors (neurofibromatosis 2) underwent a total 25 procedures. Hearing initially preserved 35% 44% those tumors. Results are reported terms pre- postoperative pure tone average speech discrimination scores. Surgical access to tumor obtained via middle cranial fossa suboccipital approaches. The latter has been used more often over past 5 years because lower associated incidence transient facial paresis. Persistent headaches have most common complication following approach. results preoperative brain-stem auditory evoked response (BAER) studies useful predicting outcome preservation attempts. Patients intact BAER waveform morphology normal or delayed latencies higher probability comparison abnormal morphology.
