Coronary artery tortuosity: a narrative review.

摘要: Coronary artery tortuosity (CAT) is a prevalent angiographic finding commonly associated with aging, hypertension, atherosclerosis and other conditions. Preliminary evidence suggests that degradation of elastin, key component extracellular matrix in the vascular wall, may be responsible for development CAT. The clinical significance CAT should considered several aspects. First, coronary flow alteration result myocardial ischemia owing to reduced perfusion pressure distal tortuous segment. Second, increased oscillatory shear stress vessel promote atherosclerotic plaque formation acute syndrome. Third, as one criteria lesion complexity, presence severe proximal culprit pose challenge wiring stent or balloon delivery, thereby increasing risk periprocedural complications. Last, serve diagnostic clue concurrent vasculopathy such fibromuscular dysplasia spontaneous dissection. In general, represents benign entity does not require specific treatment intervention. Further research warranted elucidate pathogenesis prognostic effect tortuosity.
