摘要: Results: Eighty eight percent of public health centers in this study reported that their overall quality chronic care was at a limited or basic level. About 68% the organization as reasonably good fully developed to provide care. On other hand, 96% clinical information system very support The decision support, integration Chronic Care Model components, delivery design, community linkages, and self-management were evaluated level by more than half centers, respectively. Conclusion: In metropolitan area South Korea, not found reach acceptable levels services. It is critical enhance centers.
