摘要: Background: Repair of heel soft-tissue defects remains a challenging problem in reconstructive surgery. The distally based sural neurofasciocutaneous flap is among the flaps choice for coverage this difficult region. authors describe modified lateral retromalleolar perforator-based neurocutaneous with lower pivot point. Methods: This study was divided into two parts: anatomical and clinical application. In study, 12 cadavers were injected red gelatin, all fasciocutaneous perforators between malleolus Achilles tendon (called space) identified. Clinically, on five cases reconstructed flap. Results: showed that there are usually to three cutaneous arising from terminal part peroneal artery space. Their outer diameters range 0.1 0.8 mm. A direct venous communicator, accompanied by larger perforator, connected superficial lesser saphenous vein deep venae comitantes. Five patients treated ranging 3 X 6 cm 5 cm. distal point designed at 1 above tip malleolus. All survived without complications. Conclusions: perforator predictable reliable design point, procedures simple rapid, can be rotated easily dog-ear deformity. should considered preferred reconstruction.
