Selective portal clamping to minimize hepatic ischaemia–reperfusion damage and avoid accelerated outgrowth of experimental colorectal liver metastases

摘要: Background: Temporary vascular clamping during local ablation for colorectal liver metastases increases destruction volumes. However, it also causes ischaemia–reperfusion (IR) injury to the parenchyma and accelerates outgrowth of microscopic tumour deposits. The aim this study was investigate effects selective portal on hepatocellular damage growth. Methods: Mice carrying pre-established hepatic micrometastases underwent either simultaneous both vein artery or median left lobes 45 min. Sham-operated mice served as controls. Hepatic growth were assessed over time. Results: Standard inflow occlusion resulted in a rise enzymes, inflammatory response necrosis. accelerated three- fourfold clamped compared with non-clamped (27·4 versus 7·8 per cent, P < 0·010). Conversely, induced minimal injury, tissue inflammation necrosis, completely stopped micrometastases. Conclusion: Selective does not induce accelerate micrometastasis may therefore be preferable method ablative treatment metastases. Copyright © 2006 British Journal Surgery Society Ltd.
