Through decision-making experienced published authorities in health transitions, including physicians both as well communication studies, authors propose style principles clinicians may use transition With representation from community academic settings, similarities internal are highlighted heighten appreciation values, attitudes, goals each particularly The also examine different media, behaviors, tools that share patient information. Quality measures proposed structure, process, outcome framework institutions seeking evaluate monitor improvement performance. Validation studies determine if suggested improvements result improved outcomes be necessary. By exploring dynamics suggesting best practices, hope strengthen skills contribute continuity care.
