Laparoscopic Staple-less Splenectomy: feasibility and benefits

摘要: Background: The surgical procedures of the spleen include open splenectomy (OS) laparoscopic (LS). Several techniques for control splenic vessels during LS have been developed, such as Clips, sutures, bipolar vessel sealing devices Liga-Sure and ultrasonic Harmonic have. shows some advantages if compared with other methods staplers clips shorter hospital stay, safer, feasible excellent cosmetic results. And (Liga-Sure) is cheap alternative to use endo-staplers which expensive. Objectives: study evaluate feasibility, merits demerits staple-less in cases non-traumatic disorders assess operative time, need blood transfusion, rate conversion or stapler, period stay associated complications. Patients Methods: was conducted on 32 patients, underwent using (Ligasure) pedicle control. Informed consent obtained all patients. Results: 30 patients (93.80%) were females 2 (6.3%) males. patient immune thrombocytopenic purpura (ITP) hereditary spherocytosis (HS). Two detected accessory (splenule) at hilum. converted splenectomy. To bleeding, we used stapler two (6.3%). Cost less expensive than more  by about 12000 EGP (700$). Conclusion: In a report early 1990s, dissection hilum generally achieved by endo-gastrointestinal staplers. Subsequently, new tools electrosurgical LigaSure had employed supporting device that can seal supra-physiologic burst pressures equal those ligatures. We, this series, believe it is, statistical significance, safe effective, reduces loss, minimize operating fewer costs valid. Moreover, reduced rates selected Last but not least, resulted no post-operative complications transfusion.
