This syndrome characterized by a hypermetabolic response to all commonly used inhalational anesthetics depolarizing muscle relaxants. It can happen during or after the anesthesia. We describe case of malignant orthotopic liver transplantation. Our patient was 48-year-old man, born in Brazil but European ascendance, who developed associated use isoflurane. He treated dantrolene, which caused hepatic graft dysfunction. important that transplant teams be able recognize treat this complication.
