The role of radical parametrectomy in the treatment of occult cervical carcinoma after extrafascial hysterectomy.

摘要: Abstract Objectives . To assess the morbidity and efficacy of radical parametrectomy (RP) performed following extrafascial hysterectomy in patients with occult cervical carcinoma. Methods An IRB approved retrospective chart review identified 23 that underwent RP pelvic and/or para-aortic lymphadenectomy upper vaginectomy. Data were collected on demographics, tumor stage, grade, histology, indication for hysterectomy, surgical findings, complications, recurrence, survival. Results Of patients, 2 had a stage IA lesion while 21 IB 1 lesion. There 5 grade tumor, 10 2, 4 3, unknown grade. Median age was 41 years (range 27–59). The most common (48%) CIS cervix. Four (17%) metastasis to nodes or evidence at margin time RP. Three these positive specimen received adjuvant radiation all are alive (mean follow-up 66 months). One patient declined is 42 months. 7 (30%) operative complications: Most notably blood transfusions. Two 19 (11%) no residual recurred salvaged (follow-up 103 With median 61 months 9–103), overall 5-year survival 96%. Conclusions an acceptable option diagnosed carcinoma hysterectomy. Careful selection unlikely have will obviate need instances.
