Long-term survival and sequelae after surgical management of invasive cervical carcinoma diagnosed at the time of simple hysterectomy

摘要: From 1956 to 1988, 27 women (median age, 60 years) found have occult invasive carcinoma of the cervix at total hysterectomy underwent radical reoperation consisting parametriectomy, upper vaginectomy, and pelvic lymphadenectomy. Residual disease was present reexploration in 4 (15%) patients: lymph nodes 2, parametrium 1, vagina a para-aortic node 1. All patients were followed minimum 18 months; there no deaths within 3 months operation. However, 2 (7%) developed ureterovaginal fistulas. Recurrent observed 6 (22%) had successful salvage procedures, died disease, all years reoperation. Recurrence correlated with presence residual nonsquamous histologic findings. At median follow-up 8.4 years, 23 alive disease-free. The 5-year absolute survival estimate (Kaplan-Meier) 82%. Radical can be performed safely selected who early-stage time expectation an acceptable rate long-term disease-free survival.
