Clinical characteristics and prognosis of patients with hepatoblastoma.

摘要: Hepatoblastoma is a rare pediatric malignancy that frequently appears in an advanced unresectable stage. Improved resectability and survival rate have been reported with the help of neoadjuvant chemotherapy. Between January 1987 June 1995 series 20 patients were managed diagnosis hepatoblastoma. The median age at was 13 months (2 to 7 years 10 months) male/female ratio 13:7. Chemotherapy effectively reduced tumor volume (with statistical significance: p= 0.008) able convert seven nine initially tumors (78%) resectable ones. Altogether, 14 operations done, 12 radical 2 palliative, or without adjuvant whole population followed for duration 33 months; group 26 months. curative resection displayed 5-year 61.1%, but none noncurative survived more than (p= 0.0001). In univariate analysis prognostic factors, large size absence thrombocytopenia associated poor survival, these differences not statistically significant. Pure fetal histology better prognosis. this new era chemotherapy, optimal management strategy hepatoblastoma still debated, surgical earliest possible time being final goal. For now individualized approach be protocol choice.
