The Natural History of Disability and Its Determinants in Adults with Lower Limb Musculoskeletal Pain

摘要: Objective. To investigate determinants of deterioration or improvement in disability people with chronic hip and knee pain. Methods. We analyzed data from the Somerset Avon Survey Health, a longitudinal, community-based cohort study containing collected 1994–95 again 2002–03. The Medical Outcomes Study Short-Form 36 was completed by subjects at both timepoints, used to categorize as disabled not. Baseline were explore possible change functional status over 8-year time period. Adjusted odds ratios (OR) derived multivariate, multinomial logistic model. Results. Data available on 1072 subjects, all whom reported and/or pain baseline. At baseline, 56.8% women 42.0% men disabled. Of 545 disabilities 107 (19.6%) no followup; 527 177 (33.6%) became development significantly associated older age (OR 2.1), living most deprived areas (2.4), presence 3 more comorbidities (3.6), problems physical function baseline (2.0), severe (2.4). mirrored those deterioration. suggest “threshold effect” which recovery becomes unlikely. Conclusion. presenting pain, healthcare professionals should be concerned about who are older, lower socioeconomic status, comorbidities, have pain. Much longstanding might preventable.
