摘要: Approximately one third of all drugs prescribed in the United States are considered unnecessary. Polymedicine, polypharmacy, medication nonadherence, and adverse drug reactions among top five causes acute care hospitalizations persons older than age 65. These conditions significantly increase health costs will continue to do so at an alarming rate as America's population ages. Although medications important factor improving maintaining quality life for adults, polymedicine places them risk admission medication-related problems other complications. This article reviews common errors, explores depth problem aging clients, advocates use gerontological nurse practitioners advanced practice nurses specialty-trained elder pharmacists active members interdisciplinary team.
