摘要: Management of periprosthetic fractures around the femoral stem after total hip arthroplasty (THA) represents a significant challenge and optimal treatment remains controversial. The most common paradigm involves treating well-fixed with osteosynthesis, whereas loose require revision those poor bone augmentation graft. Paradoxically, literature reports higher rate failure for osteosynthesis prostheses considered to be well-fixed. Such high outcomes may result not only from difficult fracture fixation compromised biologic healing, but also unrecognized peri-implant pathology. Therefore, proper preoperative intraoperative evaluation is key, subset patients benefit alternative management. We review appropriate methods Vancouver type B particular emphasis on avoiding missteps that can lead failure.
