摘要: Summary Background With increasing survival of patients with more severe forms congenital diaphragmatic hernia (CDH) and risk long-term respiratory morbidity, studies on lung morphology are needed. We used hyperpolarised 3He MRI anatomical 1H in a cohort young adult CDH to image regional ventilation microstructure, focusing morphological micro-structural (alveolar) abnormalities. Methods Nine left-sided CDH, born 1975–1993, were studied. Regional was imaged MRI, the apparent diffusion coefficient (ADC) computed separately for ipsilateral contralateral lungs. anatomy, total volume motion during free-breathing. Results 3He showed abnormalities six patients, ranging from single defect (3 patients) multiple defects both lungs (one patient treated extra corporeal membrane oxygenation). In eight ADC values significantly higher than those lung. Conclusions Functional changes persist into adulthood most patients. Ipsilateral elevated consistent enlargement mean dimensions confining micro-structure at alveolar level. Pediatr Pulmonol. 2015; 9999:1–10. © 2015 Wiley Periodicals, Inc.
