Prediction of occult neck metastases in laryngeal carcinoma: role of proliferating cell nuclear antigen, MIB-1, and E-cadherin immunohistochemical determination.

摘要: The aim of this study is to investigate the predictive value proliferative activity assessment and E-cadherin expression by means immunohistochemistry in identifying patients with laryngeal squamous cell carcinoma at a high risk for occult node metastasis. Thirty consecutive treated false clinically negative nodes (occult metastases, pN+) between years 1980 1990 were selected study. A group 30 cases cervical lymph (pN-) having similar anatomic site tumor size distribution was used as control. In each case, several histological parameters, including grade, pattern invasion, number mitosis (x10 high-power field), inflammatory infiltrate, sclerosis, assessed. Proliferative determined using immunohistochemical staining proliferating nuclear antigen (PCNA) MIB-1. Other putative prognostic factors investigated level adhesion molecule two oncoproteins, p53 c-erbB-2. pN+ cases, PCNA MIB-1 significantly higher than pN- group. Moreover, significant loss observed carcinomas metastases. No differences c-erbB-2 oncoproteins found cases. Among other pathological parameters examined, only grade associated presence but on multivariate analysis, relationship lost. We conclude that PCNA, MIB-1, are independent predictors nodal disease carcinoma, their determination could be useful who considerable metastases may benefit from elective neck dissection.
