When to address level I lymph nodes in neck dissections

摘要: Abstract Objective To investigate the involvement of level I neck lymph node groups in head and carcinoma compare effect primary tumor sites, such as oral cavity (OC), oropharynx (OP), hypopharynx (HP), larynx (Lx), on metastasis. Study Design Case series with chart review. Setting Comprehensive Cancer Center. Subjects Methods A total 243 patients OC laryngopharyngeal who underwent dissections last three years were included study. The site was OC, followed by OP, Lx, HP, unknown (CUP). Results Level involved 29 (11.9%) patients. other levels also positive all but five (17.2%) I–involved metastasis (19.1%), CUP (11.1%), OP (9.8%), Lx (4.4%), HP (0%). only if there multiple adverse prognostic features, N3 neck, extracapsular spread, pathologic resected nodes, I-V, invasion submandibular gland. Conclusion Although content is part radical modified dissections, I–sparing selective could be a safe effective surgical management strategy appropriately selected carcinoma.
