Responses of the skin microcirculation to acetylcholine and sodium nitroprusside in patients with NIDDM.

摘要: The mechanisms involved in the pathogenesis of microangiopathy occurring non-insulin-dependent diabetes mellitus (NIDDM) are unclear. In present study, blood flow responses to vasodilators acetylcholine (which acts via endothelium) and sodium nitroprusside (a smooth muscle relaxant) were evaluated this patient group. 14 male patients with NIDDM, treated either diet alone (n = 6) or plus insulin, (mean age 59 years) age-pair-matched control subjects, forearm skin perfusion following multiple doses iontophoretically applied 1% 0.01% was recorded by laser Doppler imaging. Basal not significantly different diabetic group compared results expressed as drug-minus-vehicle response. Acetylcholine increased (p < 0.001, analysis variance) all but vasodilatation attenuated subjects (0.86 +/- 0.09 vs 1.36 0.14 arbitrary units volts (V) respectively, at fifth measurement point, mean SEM, p 0.01). Skin 0.001) lower than (0.12 0.05 0.45 0.11 V, These data suggest that endothelial and/or function may be impaired microcirculation NIDDM.
