摘要: Although soft tissue free flaps have been in the mainstream for over 40 years, muscle perforator per se are a relatively recent addition to armamentarium of reconstructive microvascular surgeon. Even though actually only fasciocutaneous flap subtype, distinctively different approach is necessary their safe and reliable use, which has deterred many from adopting this valuable asset fear not being able master an implied "learning curve." Whether justifiable excuse led our examination original microsurgical experience 1982-1986, retrospect had its own learning curve. All 30 during that initiation period were flaps, now unacceptable 37% major complication rate but also complete failure 26% due specifically technical inadequacies with requisite microanastomoses. When compared first there was similar incidence complications (30%), although eventual transferred success 97%. This confirmed existence curve preliminary consistent any surgical innovation. However, prowess by time facilitated acquisition skills comfortably harvest very acceptable minimized steepness particular Just what will be configuration unavoidable specific each individual depend on capabilities, relative difficulty given flap, level competency expected.
