Comparison of hemodynamic effects and plasma cyclic guanosine monophosphate of nicorandil and nitroglycerin in patients with congestive heart failure

摘要: Hemodynamic tolerance has been observed within several hours of continuous infusion nitroglycerin (NTG). We examined the hemodynamic parameters as well femoral arterial and venous cyclic guanosine monophosphate (cGMP) concentrations during intravenous NTG or nicorandil, a nitrate potassium channel opener, in patients with congestive heart failure. Doses nicorandil were titrated to achieve > = 25% reduction pulmonary capillary wedge pressure (PCWP) 1 hour, was maintained at constant rate for 24 hours. The PCWP mean blood identical after 1-hour either nicorandil. In group, not significantly different from baseline value 12 hours, but group remained lower than preinfusion cGMP production (assessed by difference between plasma level) paralleled changes PCWP, suggesting that arteriovenous is biochemical indicator tolerance. Although sustained decrease also attenuated infusion. These findings suggest absence likely due its action activity.
