Transient left ventricular apical ballooning: A unifying pathophysiologic theory at the edge of Prinzmetal angina

摘要: Introduction: Takotsubo or ampulla transient left ventricular apical ballooning (LVAB) cardiomyopathy has been described as a clinical syndrome characterized by the sudden onset of chest pain, cardiac failure, ischemic changes on electrocardiography, and severe myocardial dysfunction in presence “normal” coronary arteries angiography. All features last from few days to weeks. Methods Results: On basis previous knowledge light our recent experience with acetylcholine testing this condition, cases four patients are preliminary but sound arguments given support theory that LVAB is caused severe, sustained spasm many all vessels. In one these patients, experimental reproduction occurred catheterization laboratory during (as evidenced echocardiographic monitoring), while two other test provoked similar, extensive angiographic vasospasm suggestive symptoms. addition, similarities differences exist between Prinzmetal angina discussed. Conclusion: To evaluate pathogenesis LVAB, it proposed be routinely performed under specific, prospective, investigational protocols at specialized centers. © 2008 Wiley-Liss, Inc.
