Undifferentiated (spindle cell) pancreatic carcinoma: a case report with osteochondroid differentiation.

摘要: Context Undifferentiated (spindle cell) carcinomas of the pancreas are rare anaplastic variants pancreatic ductal adenocarcinoma with a frequency 2% exocrine tumors. Their clinicopathological features limited and obtained by few previously case reports. We report undifferentiated carcinoma focal osteochondroid differentiation. Case A sixty-six-year-old woman was admitted to our hospital for abdominal pain nonspecific nausea almost 40 days. Imaging studies revealed well-defined cystic–solid mass heterogeneous density involving tail pancreas. performed an en bloc distal pancreatectomy splenectomy radical excision, as well regional lymphadenectomy. The resected specimen 4.0×5.0 cm exophytic clear-bordered neoplasm containing necrotic calcified areas, without splenic invasion. lymph node involvement not detected (0/5) surgical margins were negative. Microscopy showed pleomorphism giant cells, spindle-shaped cells anaplasia, diagnosis differentiation made. patient declined chemotherapy extended She suffered from liver nodes metastasis 9 months after surgery, she subsequently died 4 later due high tumor burden. Conclusions is but associated extremely poor prognosis. It should be included in differential lesions. Image: O ssification edge.
